Online Request Form

Just click "Online Request Form" above and fill out the form accurately and completely. An automated email confirming the details of your request is always sent to your Lotus Notes account, so if you do not receive it, an error has kept your request from reaching us.

If you wish to browse, search, or request a VHS, DVD, or CD-ROM from the ITC's multimedia collection, just click "Media Catalog" below.  Once you select your media, you will be automatically directed to send the request using the online request form.

Please be aware that the automated email you receive only confirms that we have received your request. It does not necessarily mean your request can be met. In order to provide the best service, we carefully and methodically enter each request into the scheduling software by hand. This allows us to catch many errors and problems immediately, but does take time. For this reason, we ask that you always make your requests well in advance of your class time (at least 24 hours is recommended). That way, if there are questions about your request or a scheduling conflict, a helpful ITC student assistant will have time to contact you by phone or e-mail in order resolve any potential complications prior to the start of your class.
